How this lineup is built
The highest Care & cost score belongs to SnagRx, at 94. Four checks add up to that 0 to 100 score: price is worth 35, care 25, pharmacy 25 and clarity 15. The scoring method spells out each one. Semaglutide's price band is $90 to $300 a month. Charge $90 or less and a provider keeps all 35 price points. Charge $300 or more and it gets none. Halfway, around $195, is worth about half.
The band is set once per drug and changes only with a dated method update, so nobody's points drift because a rival cut prices. Many semaglutide plans price each dose step on its own, so the figure here is where you start, not where you'll land. The compounded semaglutide cost guide covers the rest.
Line up semaglutide plans by what stays true after month one
Compare both the dispensed preparation and the service around it: how often you're billed, whether a dose change costs extra, and whether the provider says who makes the drug.
Read each card left to right. The score tells you how a provider did on all four checks, the chip row shows where the points came from, and the price is the month-one price. Two providers with the same score can get there differently, one through price and one through care and a named pharmacy.
When two prices are close, let the Pharmacy chip break the tie. A pharmacy you can name is a pharmacy you can look up with your state's board of pharmacy.
Coming from brand Ozempic or Wegovy? Ask how the provider sets a starting dose for someone already on semaglutide. The price assumes a new patient at the starting dose.
The chip row also shows what a provider left out. A Clarity chip under 15 means its price, its terms or its state list wasn't on its pages before sign-up.
How the four checks play out on semaglutide
- Price (35). The month-to-month price at the starting dose plus any membership. Prepaid multi-month plans appear on reviews but are never scored, since paying ahead isn't a monthly price.
- Care (25). Who prescribes, a clinician's review of your intake, labs and who pays, a follow-up rhythm, and dose changes with no new consult fee, at five points apiece. Semaglutide's gradual climb in dose makes the follow-up check worth reading closely.
- Pharmacy (25). Ten points for naming the pharmacy, five for its state or 503A/503B status, five for saying plainly that the product is compounded, and five for never calling it FDA-approved.
- Clarity (15). Five points each for a price you can see before the intake, published cancellation and refund terms, and a list of states served or a plain all-50 statement.
What the semaglutide prices add up to
On this lineup, starting-dose prices run $75 to $349, a yearly difference of about $3,288 by our math. The middle price, $199, works out to about $2,388 a year. These are annualized comparisons at unchanged starting prices, not the actual payments due in the first 365 days; dose changes, introductory offers and separate tests can change the budget.
Across every program we track, not only the 26 on this lineup, 25 offer compounded semaglutide at $75–$349 a month, with a median of $199. Brand Wegovy through telehealth programs runs $414–$498.
Within semaglutide's band, every ten dollars off the monthly price adds about 1.7 price points. A much lower price can still lose to a provider that is open about its pharmacy and its care.
A verified four-week charge has a different annualized cost from a calendar-month charge. The comparison uses a 365-day year; an enrollment payment followed by 28-day renewals creates 14 payments in the first 365 days. A four-week medication supply alone does not establish that billing interval.
Paying ahead for several months can lower the monthly figure, but you carry the risk of stopping early. The score never counts a prepaid price, so the prices stay comparable from one provider to the next.
How to read this lineup
- Read the score first, then the price: the bold figure is the Care & cost score, and the dark box holds the starting-dose price per month.
- If a provider bills every four weeks, its price is restated per calendar month so you can set it fairly against monthly billers.
- Rows tagged Partner come from providers that may pay us when you sign up through our link. The tag brightens the button; it can't lift a row past a higher score.
- When two providers tie, the one with more price points goes first. If they match on both, a partner goes ahead, and past that the alphabet decides. The partner box above the list sits outside this order.
- A price reading “No monthly price to compare” (1 on this lineup) means the provider's own pages don't give one month-to-month figure at the starting dose, so its price part scores zero.
Before you pay for compounded semaglutide, ask
- Is the price the same at every dose, or does each step cost more?
- Does the vial contain anything besides semaglutide, such as B12, and how much?
- Which pharmacy compounds it, and will its name be on the label?
- If you choose a multi-month plan and stop early, what do you get back?
- Who do you message about side effects, and how fast do they answer?
- Do you need recent labs before the first prescription, and can you send your own?
Questions about this lineup
- How much is compounded semaglutide a month?
- Among the 25 providers here that print a month-to-month figure, the starting dose costs $75 to $349, with $199 as the midpoint. Required memberships are already added. Ask what the next dose step costs, because many plans price each step separately.
- Is compounded semaglutide the same as Ozempic or Wegovy?
- The molecule matches; the product doesn't. Ozempic and Wegovy are made by Novo Nordisk and approved by the FDA, while a compounded version is made by a pharmacy without that review. Some versions add vitamin B12, and the pharmacy label is the only place to see how much.
- Can you still get compounded semaglutide after the shortage?
- Yes, through a prescriber and a 503A pharmacy, case by case. The shortage-era allowance ended in spring 2025, so a pharmacy may fill it for an individual patient but not as routine copies of the brand.
- Why can a higher price still rank above a lower one?
- Because price is only 35 of the 100 points. Naming the pharmacy alone is worth 10, and the care checks are worth 25. A provider that is clear about who prescribes and who fills can outscore one that charges less and says little.
The Care & cost score
Every provider gets a 0 to 100 Care & cost score for each drug it prescribes, built from four checks: what a new patient pays per month at the starting dose (35), how care works (25), whether the pharmacy is named (25) and how clearly prices and terms are shown (15).
