Insurance cost breakdown
Separate your drug cost sharing from a clinic membership using figures supplied by your insurer.
Your written plan terms
Use figures from your insurer. Enter zero only when a charge does not apply.
Enter your plan figures
Complete all five fields with nonnegative figures; coinsurance must be 0–100%.
This model applies the deductible first, then coinsurance, then an additional copay, capped at the allowed drug amount. Many plans use a copay instead of coinsurance. Enter zero for the unused field. It excludes coupons, coverage limits, out-of-pocket maximums, and medical-benefit billing. It cannot establish coverage or predict an insurer's final claim.
