How this lineup is built

To make this list, a program's month-to-month price for compounded semaglutide at the starting dose, plus any membership it requires, has to land below $200. Inside that line, programs keep the order and the scores they hold on the full semaglutide lineup. Nothing is rescored here, and no program is moved up for being lower priced.

Semaglutide's price band starts at $90, and a price at or below it collects all 35 price points (how we score). Each ten dollars below the band's top is worth about 1.7 points, so prices this low already hold most of the price credit on offer. What moves a program up or down here is care, pharmacy and clarity.

One fact holds at any price: compounded semaglutide has no FDA approval, and compounded preparations do not undergo FDA premarket review for safety, effectiveness or quality; pharmacy regulation and testing remain separate matters.

At this price, the fine print does the sorting

Semaglutide is one of the most heavily discounted drugs online, and a lot of that discounting lives in banners. Struck-through “regular” prices, seasonal sales and “lifetime price” promises all appear on programs that made this list.

None of those words change a price. The price is the price you'd pay in month two on a plan you can cancel month to month. A struck-through figure next to it is a marketing anchor, not a saving you can bank.

So when two prices are close, open both reviews and read what the program commits to. Some promise the same price at every dose in writing. Others say the figure is a starting price that “may vary based on dosing.” The first kind is worth more than a few dollars of difference.

Check the minimum term, too. One program here states a two-month minimum in its terms, and another bills a yearly membership a few weeks after you start. Neither shows up in a monthly price, and both decide what leaving costs.

What the price counts, and what it can't

  • Counted: the membership. A yearly fee is divided by twelve and added to the monthly price, so a program with a small drug price and a yearly membership is compared on the total.
  • Counted: the verified charge interval. A 28-day charge is annualized using 365 divided by 28 and then by twelve. Enrollment followed by uninterrupted renewals creates 14 first-year payments, rather than that annualized cash equivalent.
  • Not counted: prepaid terms. A 6- or 12-month plan charged up front often prints the lowest number on a program's page. The price uses the month-to-month price instead.
  • Not counted: first-order discounts. A struck price on your first order or a first-month code is left out, since month two is the price you live with.
  • Can't count: later doses, labs and add-ins. A price is the starting dose only. Whether labs are billed separately, or whether the vial adds B12 or B6, shows on the review, not the price.

What under $200 buys over a year

Prices here start at $75 a month, and the middle one is $165, about $1,980 over twelve months by our math. Across every program we track, 25 list compounded semaglutide at $75–$349 a month, with a median of $199. These are annualized comparisons at unchanged starting prices, not the actual payments due in the first 365 days; dose changes, introductory offers and separate tests can change the budget.

The brand comparison is closer than it used to be. Through the programs we track, brand Wegovy runs $414–$498 a month, and the Wegovy pill alone runs $298–$350. NovoCare's own self-pay prices, listed on the Wegovy guide, start lower for the first fills and rise after.

That makes the dose path matter more than the first bill. The Wegovy label starts the injection at 0.25 mg a week and climbs over several months, and a compounded plan that prices each step can cost well over $200 by the time you reach a maintenance dose.

If you plan to stay a year, compare a flat-price plan's twelve-month total with a dose-priced plan's total at the dose you expect to reach. The compounded semaglutide cost guide has the later-dose prices.

How to read this lineup

  • Each program here costs less than $200 a month at the starting dose with required fees added. Prices round up to the whole dollar, so a price a cent under the line can print as the line itself.
  • Scores and their four parts are carried over unchanged from the full semaglutide lineup.
  • Programs with a Partner tag may pay us if you start care through our link. The tag changes the button only, and the labeled Partner box above the list sits outside the order.

Ask these before a low semaglutide price wins

  1. Is the price I see a sale price, and when does the sale end?
  2. Does the same price hold at every dose, and is that written in the terms?
  3. Is there a minimum number of months, or a fee to cancel early?
  4. When is the membership charged, and is it refundable?
  5. What else is in the vial, such as B12 or B6, and how much?
  6. Are labs needed before the first prescription, and who pays for them?

Questions about this lineup

Where can you get compounded semaglutide for under $200 a month?
From the 15 programs listed here, which charge from $75 a month at the starting dose with required fees included. Each review lists the program's other doses and plan lengths, so check the price at the dose you expect to reach.
Is compounded semaglutide under $200 still the same drug?
It's the same active ingredient as Ozempic and Wegovy, made by a compounding pharmacy rather than Novo Nordisk, and it isn't FDA-approved. Some versions add B12 or B6. A low price doesn't tell you which pharmacy made it, so look at the Pharmacy chip.
Why does a program advertise a lower price than its price here?
Most often the advertised price is a 6- or 12-month prepaid rate, a sale price on the first order, or a drug price with a membership billed on top. The price uses the month-to-month price with the membership added in.
Is brand Wegovy ever under $200 a month?
Sometimes, at the start. Through telehealth programs, the Wegovy pill runs $298–$350 a month and the brand overall $414–$498. NovoCare's self-pay prices are lowest for the first fills and first doses, and rise after that.

The Care & cost score

Every provider gets a 0 to 100 Care & cost score for each drug it prescribes, built from four checks: what a new patient pays per month at the starting dose (35), how care works (25), whether the pharmacy is named (25) and how clearly prices and terms are shown (15).
