Compounded semaglutide prices: what you'll pay each month

What each online program keeps charging after month one, set next to the Wegovy pen and pill prices Novo Nordisk now sells direct.

The short answer

Across online programs, compounded semaglutide runs $75–$349 a month as of this update at each program's ongoing price. NovoCare’s Wegovy pill starts at $149 per 30 tablets, with a higher maintenance-dose price; injection offers use four-pen supplies. First-month offers and prepaid plans make many advertised prices look lower than what you'll pay.

Key takeaways

  • Compounded semaglutide runs $75–$349 a month right now; the median program charged $199.
  • NovoCare sells the Wegovy pill at $149 to $299 a month by dose, and the Wegovy pen at $349 after two $199 intro fills.
  • Do not assume insurance covers a compounded drug; confirm the exact product and plan rules before budgeting.
  • A price difference does not establish equal quality, clinical effectiveness or a business’s production costs.
  • Drops, lozenges and dissolving tablets need their own formulation-specific evidence and complete offer comparison.

Compounded semaglutide cost $75–$349 a month across the online programs in the table below as of this update. Every figure is the ongoing price: required memberships are added, four-week billing is stretched to calendar months, and first-month deals are left out of the headline. For what the drug is and where it stands legally, see the compounded semaglutide guide.

Compounded semaglutide skips FDA approval: it does not receive FDA premarket review for safety, effectiveness or quality. This is distinct from pharmacy licensing and other regulatory oversight [1]. Keep that in mind when a low price looks like the whole story.

What each program charges right now

The table compares selected current offers with links to full reviews. A monthly equivalent is a comparison rate; prepaid charges and fixed-day renewals can produce a different payment schedule. Weekly injection plans only; non-injection forms have their own table further down.

Prices from each provider's own site. Real monthly cost includes any required membership and turns 4-week billing into calendar months.

An annual-only offer or an individually quoted prescription should be evaluated separately from a flexible monthly subscription. Ask for the complete paid commitment rather than infer it from a monthly headline. Low-dose plans have their own quantities and assumptions; see microdosing GLP-1s.

Novo Nordisk's own prices have narrowed the gap

The Wegovy pill costs $149 a month at 1.5 mg through NovoCare, and the Wegovy pen costs $349 a month once the intro fills run out [2]. The pill offer defines a month as 30 tablets, while the injection offer defines it as four single-dose pens. These are supply units rather than a promise of calendar-month coverage. New pen patients pay $199 for each of their first two monthly fills at 0.25 or 0.5 mg, an offer that runs through December 31, 2026.

Wegovy pen0.25 to 2.4 mg$349/moFour-pen supply after introductory eligibility
Wegovy HD pen7.2 mg$399/moSelf-pay

Prices from NovoCare’s savings page, subject to eligibility and changes. Tablets: 30 per bottle; injections: four pens per offer supply. Starting-dose prices are not maintenance-dose quotes. Pricing is scheduled for review after December 31, 2026.

NovoCare sells the drug, but a prescriber still has to write the prescription. Clinical care and laboratory work may involve separate charges that are not included in the manufacturer’s medication offer. Confirm prescribing, product and offer eligibility separately from any membership. The pill itself is explained in our oral semaglutide guide.

Why price alone cannot establish equivalence

Different presentations and service bundles carry different prices. A vial, an injection pen and an oral tablet are not interchangeable cost units, and the comparison does not establish each manufacturer’s production costs or margins. FDA notes that compounded GLP-1s can involve multi-dose vials and measurement concerns [1]. A lower price does not establish equal strength, purity or clinical effectiveness; compounded products do not receive FDA premarket review.

That's also why FDA lists "deep discounts or prices that seem too good to be true" among its red flags for telehealth sellers [1]. The safety side is covered in is compounded semaglutide safe.

Month one rarely predicts month two

Compare a promotion’s first payment with its regular renewal charge and required commitment. An offer may apply only to new patients, a particular formulation or a prepaid package. Retain the actual checkout terms so a lower introductory display does not become an assumption about every future refill.

For confirmed 28-day billing, payment at enrollment plus renewals through day 364 means 14 payments within the first 365 days. The last payment funds supply beyond that window. A different calculation annualizes recurring cost: multiply by 365 and divide by 28, then divide by 12 for a monthly equivalent, about 8.6% above the per-cycle price. A four-week supply alone does not establish 28-day charges.

Ask which prescribed doses the quoted price includes

Wegovy’s labeled injection escalation begins at 0.25 mg, while its tablets begin at 1.5 mg and use 25 mg as the adult weight-management maintenance dose [3]. A low starting-dose offer therefore does not establish a long-term maintenance budget. Do not transfer these branded schedules to a compounded preparation or change a prescribed dose to fit a price. Ask the program how the charge changes with the actual formulation, quantity and clinician-directed prescription.

Prepay, memberships and what the price covers

SnagRx’s advertised annual package involves a different cash commitment from its monthly plan. Collective’s required annual membership is spread across twelve months for comparison, while Live Vital advertises a total for a ten-week protocol. Belle’s newer help pages describe full-plan upfront payment options and continued prepaid shipments after cancellation. These structures do not create an automatic unused-month refund. Ask what happens if a clinician changes treatment or stops prescribing before the paid term ends.

What the price covers varies too. Check the lab charge, prescriber fee, medication, supplies and shipping separately. A price that includes visits can still leave required lab work outside the quoted total. To test a prepaid plan against a monthly one, plug both into the monthly cost tool.

Insurance, HSA and FSA: what to expect

Do not assume that your insurance covers compounded semaglutide. The online programs are built around cash pay, and Collective says it does not bill insurance. Brand Wegovy is a different story: NovoCare says eligible Medicare patients pay $50 a month for Wegovy through the Medicare GLP-1 Bridge program, for weight loss only, with eligibility set by CMS [2].

Paying from a health savings or flexible spending account turns on two IRS rules. A drug that needs a prescription qualifies as a medical expense, and weight-loss treatment qualifies when it's for a disease a doctor has diagnosed, with obesity among the IRS's own examples [4]. An HSA uses that same definition of medical care [5]. These general rules do not establish every bundled charge as eligible or guarantee reimbursement for a specific compounded prescription. Confirm the itemized product and clinical expenses with the benefits administrator; card acceptance alone is insufficient. Keep receipts and avoid treating a tax-related account as insurance coverage.

Drops, lozenges and tablets aren't reliably cheaper

Compounded drops, lozenges and dissolving tablets are separate preparations. Their route, delivered exposure and clinical evidence need evaluation independently of an injection price. Wegovy tablets use a defined absorption-enhancing formulation; its approval does not validate a pharmacy’s different oral or sublingual product. Compare the exact formulation and supply, and ask for product-specific evidence rather than assume an oral format is equivalent or cheaper.

Read the conditions behind a monthly equivalent

A published monthly equivalent is one part of the decision. Before accepting the offer, check any membership folded into it, whether the compounding pharmacy is named, and the states it ships to. The providers are scored on more than price on the compounded semaglutide scores, and the sign-up steps are in how to get compounded semaglutide. If you're pricing tirzepatide too, it has its own cost page.

Frequently asked

How much does compounded semaglutide cost a month?
Expect $75–$349 a month, the current range across online programs, counting each program's ongoing price with required memberships included. The median was $199. An introductory charge has its own qualifying and renewal conditions.
Does insurance cover compounded semaglutide?
Ask your plan about the exact formulation, dispensing pharmacy and clinical indication. A self-pay program does not establish coverage, and a benefits card is not proof of insurance reimbursement. Check covered branded alternatives and prior-authorization requirements separately.
Why is compounded semaglutide so much cheaper than Ozempic?
Different products and service bundles have different prices; this comparison cannot establish their production costs or profit margins. Compounded products do not receive FDA premarket review, and a lower price does not demonstrate equal quality. Compare exact quantities, renewal conditions and clinical fees.
Is the Wegovy pill cheaper than compounded semaglutide?
A starting-dose price can be lower, but it is not a maintenance-dose budget. NovoCare charges $149 a month for the 1.5 mg pill and $199 for 4 mg, which falls inside the compounded range of $75–$349. The 9 mg and 25 mg pills cost $299 a month, and you still need a prescriber.
Are cheap compounded semaglutide offers safe?
A price can't tell you whether a product is made well. Look for a program that names its compounding pharmacy, screens you with a licensed prescriber, and stays reachable after delivery. FDA's own red-flag list for telehealth buyers includes prices that seem too good to be true.
