Ozempic's cash price is well below its list price. Novo Nordisk now sells it directly through eligible self-pay offers, with the price depending on the package and strength [1]. The offer defines its pricing unit as one box containing one pen, which is not automatically a calendar month of treatment. What changes the number is your dose, if you're a new patient, and your insurance. The drug inside Ozempic is covered in semaglutide, and the brand in Ozempic.
Ozempic costs $349 to $499 a month in cash, by dose
Novo's current self-pay offer lists $349 for a one-pen box at 0.25, 0.5 or 1 mg and $499 at 2 mg. Patients new to the savings offer or NovoCare Pharmacy may qualify for $199 for each of two introductory fills at 0.25 or 0.5 mg through December 31, 2026. A valid prescription and the offer's eligibility terms apply; Novo can modify or cancel the program. The pricing guide also distinguishes local-pharmacy pickup from home-delivery availability [1] [2].

Novo Nordisk says its self-pay pricing will be “updated after December 31, 2026.” Prices change; confirm at checkout.
Twelve purchases at $349 total $4,188; twelve at $499 total $5,988. Those are package budgets, not guaranteed annual treatment costs. Pen contents, prescribed use and refill timing determine coverage. If an eligible patient buys two $199 introductory boxes followed by ten $349 boxes, those twelve purchases total $3,888. This example does not predict the patient's prescription or guarantee today's offer for a full year.
Ozempic pills start at $149 a month
Ozempic is also available as a once-daily tablet. The pricing guide lists the tablet offer for eligible local-pharmacy pickup, rather than promising home delivery. Self-pay prices are $149 a month at 1.5 mg, $199 at 4 mg and $299 at 9 mg, where one bottle of 30 tablets counts as a month [2]. Pill and pen doses don't line up milligram for milligram, so the pill's numbers aren't directly comparable to the pen's. How semaglutide pills differ from the shots is covered in oral semaglutide.
Why you'll still see $1,000-plus quoted
The figures around $1,000 come from list prices, not what a cash buyer pays at NovoCare. TrumpRx, the government's drug-price site, shows Ozempic at a $199 “Lowest Presidential Deal” next to a $1,028 “Original Price” [6]. That $199 matches Novo's new-patient starting price, and the $1,028 is TrumpRx's own label for the higher figure. Some older articles quote list prices from earlier years, which don't reflect the cash route.
A pharmacy's own cash price can differ from NovoCare's, which is why it pays to ask before the prescription is filled.
Check the exact product, indication and plan requirements
The diagnosis and exact formulation matter, but an approved indication does not guarantee payment. Ask the plan whether the prescribed Ozempic product is on its formulary, whether prior authorization or step therapy applies, and which pharmacy can dispense it. Ask for the deductible, copay or coinsurance that applies to this fill. A general statement about an insurer cannot establish the terms of a specific employer or individual plan.
Coverage for weight loss, PCOS or prediabetes is a different matter. Those aren't Ozempic indications. The semaglutide product approved for chronic weight management is Wegovy, and its prices are in what Wegovy costs. If you're paying for weight loss yourself, the monthly cost calculator turns any plan's billing into a monthly figure.
With insurance, the savings card can bring Ozempic to $25
For eligible commercially insured patients with coverage, Novo advertises a payment as low as $25, with maximum savings of $100 per defined month. That is a floor under eligible conditions, not everyone's final bill. For example, a $200 patient charge minus the maximum $100 benefit still leaves $100. The card does not guarantee that a plan will approve treatment [1].
The commercial-insurance discount excludes government beneficiaries [1]. This differs from the separate self-pay offer: the current page says government-plan members can pay the eligible self-pay price outside insurance. Those purchases cannot be submitted for reimbursement or counted toward the plan deductible or out-of-pocket requirement.
Medicare covers Ozempic through Part D, at your plan's price
Novo reports broad Medicare prescription-plan coverage, but that does not establish an individual plan's approval [3]. What you pay depends on your Part D plan's tier and where you are in the year. Novo points Medicare patients to the Medicare Prescription Payment Plan, which spreads eligible Part D out-of-pocket payments across the year without reducing the total cost. The full Medicare picture is in does Medicare cover Ozempic.
You may get Ozempic free if you're uninsured with a low income
Novo Nordisk's Patient Assistance Program provides eligible medicine without a program registration or monthly participation fee [3]. Its 2026 rules require you to:
- be a US citizen or legal resident
- have a qualifying household income; for Ozempic, uninsured patients must be at or below 200% of the federal poverty level
- meet the Ozempic-specific uninsured requirement; the page’s general Medicare eligibility language applies to other program products
- not be enrolled in, or qualify for, Medicaid, Medicare's Extra Help program or VA benefits
Ask for the cash price, and compare it with NovoCare's
Before a pharmacy fills Ozempic without insurance, ask its cash price and compare it with NovoCare's $349 or $499. Discount-card prices vary by pharmacy and change often, so check the price shown for your pharmacy rather than a figure you've read. A card may beat list price but still land above NovoCare's direct price.
The reviewed FDA record for Apotex ANDA 220314 lists tentative approval on April 7, 2026, rather than final marketing approval. That record does not establish every current generic application or local availability [9]. Getting Ozempic without insurance still takes a prescription. The prescriber must be authorized to treat the patient and determine whether the product is appropriate. Confirm the selected pharmacy and whether the offer covers pickup or delivery. Compounded semaglutide is a separate, pharmacy-made product that isn't FDA-approved; its costs are in compounded semaglutide cost, and how it compares with the brand is in compounded semaglutide vs Ozempic.
