Metformin was first approved in the US in 1995 and is now sold as a generic [1]. An online visit still requires an appropriate clinical assessment. Longevity, weight or PCOS use is off-label, and having type 2 diabetes does not by itself establish that metformin is appropriate. Kidney function, other conditions and interacting medicines matter. What metformin is and does is covered in metformin.
Yes, you need a prescription for metformin
Metformin is a prescription-only drug in the US. Its label and packaging are marked “Rx only,” so a licensed clinician has to prescribe it and a licensed pharmacy has to fill it [1]. It isn't sold over the counter.
A clinician must be authorized to treat the patient in their location and assess whether a prescription is appropriate. That includes your own doctor, an online visit, an urgent-care clinic or a membership program. A same-day appointment does not guarantee a same-day prescription or remove the need for records, laboratory testing and continuity of care.
FDA approves metformin only for type 2 diabetes
The reviewed Laurus label covers blood-sugar control alongside diet and exercise in type 2 diabetes. Its immediate-release tablets include adults and children aged 10 and older; its extended-release tablets cover adults. Age indications and instructions should be checked for the exact dispensed product rather than assumed identical across every metformin formulation [1].
The Diabetes Prevention Program randomized 3,234 adults with elevated fasting and post-load glucose. Over an average 2.8 years, diabetes incidence was 11.0, 7.8 and 4.8 cases per 100 person-years with placebo, metformin and lifestyle intervention. Relative incidence reductions were 31% with metformin (95% confidence interval 17–43%) and 58% with lifestyle intervention (48–66%) [2]. Preventing diabetes still isn't an approved use on the label.
Without diabetes, metformin is off-label
Prescribing metformin for longevity, weight or PCOS is off-label. Off-label prescribing is distinct from FDA approval for that purpose. The treating professional must assess the rationale, relevant evidence, alternatives and patient-specific risks within applicable prescribing requirements. They can also say no, especially if your kidney function or other risks argue against it.
Evidence relevant to longevity includes preclinical studies, observational associations and smaller human studies of intermediate outcomes. These do not establish that prescribing metformin extends human lifespan. A 2016 article described the proposed TAME research program, rather than reporting completed longevity results [3]. The full evidence, including what it does to exercise gains, is in metformin for longevity. For PCOS and weight, the label says nothing, and this page makes no claim either way.
What an online prescriber will check first
Expect a health questionnaire, a list of your medicines and a recent kidney-function test. The label is specific: “Prior to initiation, assess renal function with estimated glomerular filtration rate (eGFR)” [1]. Some programs order the test for you; others ask you to bring results from your own doctor.
| Check | What the label says |
|---|---|
| Kidney function | Assess eGFR before initiation and periodically afterward; the label calls for at least annual assessment, more often in patients at increased risk. |
| Severe kidney impairment | Use is contraindicated below eGFR 30 mL/minute/1.73 m2. |
| Starting treatment | Initiation is not recommended at eGFR 30–45 mL/minute/1.73 m2; this differs from reassessing an existing prescription if function declines. |
| Blood counts and vitamin B12 | The label calls for annual hematologic monitoring and vitamin B12 assessment at 2–3-year intervals, with management of abnormalities. |
| Iodinated contrast imaging | Certain patients need a clinician-directed interruption and reassessment; do not assume the same rule applies to every scan. |
| Alcohol and other medicines | Excessive alcohol and some drug interactions increase lactic-acidosis risk and need review. |

The reason for all this caution is the label's boxed warning: lactic acidosis, a potentially fatal buildup of lactate associated with metabolic acidosis. The label lists its risk factors as kidney problems, some other drugs, age 65 or older, contrast scans, surgery, low-oxygen states, excessive alcohol, liver problems and mitochondrial diseases [1]. Your prescriber will ask about each.
Four ways to get a metformin prescription
Possible routes include an established treating clinician, a separate telehealth appointment or an ongoing care program. Ask whether the service offers assessment for your actual goal, whether it accepts outside laboratory results, and where the prescription would be filled. A combination containing metformin is a different product from metformin alone and needs separate assessment.
Sesame’s current metformin page describes choosing a provider for a video visit, with a prescription sent to a pharmacy only if appropriate. AgelessRx’s product page advertises metformin for metabolic and longevity goals; those marketing claims should not be mistaken for approved longevity benefits. Found also lists metformin for off-label weight care. These are provider descriptions of access, not confirmation that a particular reader will qualify [9] [8] [7]. The Found review explains its separate service and billing terms.
Before booking, ask who will review the results and provide ongoing care if kidney function changes or side effects develop. Ask how the service communicates with an existing diabetes or primary-care clinician to avoid duplicate or conflicting prescriptions. The first-appointment guide helps organize records and questions. Related off-label evidence is discussed in metformin for longevity and rapamycin access.
What metformin costs online
Request separate quotes for the exact generic product, clinical assessment, membership and laboratory testing. A program price does not reveal how much pays for the medication or clinician. Ask whether refills, follow-up messages and repeat tests are included, what renews automatically, and what is refundable if the clinician declines to prescribe.
For a retail fill, ask your pharmacy for its cash price or check a pharmacy price tool, since prices vary by pharmacy. To turn any plan's billing cycle into a monthly cost, use the monthly cost calculator.
Fill it at a pharmacy you can trust
Use a state-licensed pharmacy. FDA's BeSafeRx page explains how to recognize a safe online pharmacy and the warning signs of sites that only look like one, and it links to a lookup for state-licensed online pharmacies [6]. A site that sells metformin without a prescription is a warning sign in itself.
When metformin isn't the right call
The label rules metformin out for people with severe kidney impairment (eGFR below 30), with acute or chronic metabolic acidosis, including diabetic ketoacidosis, or with a prior allergic reaction to metformin [1]. Starting it isn't recommended at eGFR 30 to 45.
There's also a trade-off for active people. One trial randomized 53 older adults during 12 weeks of aerobic training. Metformin attenuated average improvements in insulin sensitivity and VO2 max, but responses varied substantially [4]. The MASTERS trial randomized 94 adults aged 65 and older during 14 weeks of supervised resistance training. Placebo participants gained more lean and thigh muscle mass; the difference in strength gains did not reach statistical significance [5]. These small, specific trials do not predict every person’s response or establish that exercise loses its benefits. If staying fit is part of your longevity plan, raise this with your prescriber. More is in metformin for longevity and rapamycin.
